This is a Japanese music-themed adventure game designed by Hideyuki Nozawa of Southern All-Stars.

While the Pippin version was already very rare, and already worked without issues on Macs, this is the only copy I've ever seen of the Mac-only release. As far as I know, this is the only record of its existence on the internet.
