That could be it, but looking at the scan of the back cover I noticed that, alongside the copyrights for Software Toolworks and Arrow Micro-Techs, there was also a copyright for IMS, and that this version itself came out in 1992, a couple of years after the original DOS version of Life and Death 2 and about a year after Surgeon 3. Not only that, but the fact that this and Surgeon 3 have such different art styles compared to any other version of Life and Death 2 makes me think that Surgeon 3 came first and then AMT decided to turn it into L&D2 for this Japanese release. It's really weird.

Not odd at all, mobygames is far from a definitive resource. I own many, many games that can scarcely even be found on google, let alone mobygames.

There has to be, but what's odd is that the MobyGames page for the game doesn't even list the Mac as a released platform:

Certainly similar. Just guessing, maybe ISM Inc. purchased the rights to producing it for the US market? This Japanese version pre-dates Surgeon III by a few years. - But then I see an even earlier "The Surgeon" by ISM...
