This is the widely-redistributed beta version of the ill-fated MacBASIC from 1985. Microsoft bought the rights to it and discontinued it prior to release. Despite never passing the beta stage, Dartmouth University used it in an introductory programming course, and 2 books on the program were published that sold well for several years. Interest eventually died, however, because MacBASIC is not 32-bit clean.

Added BYTE magazine article -> Manual
The person to thank is not the uploader, but me. This came from my MacBASIC 0.335 disk. I have a photo of the disk on Mac GUI. It's been there since 2009.
This is an old version. There's a newer version (1.0) around, too.
